The entire bill is problematic and will fail in practice if implemented, so I cannot single out any specific concern alone. Rather work with the private sector to see how a sustainable approach can be created that better meets the objectives of providing heath care for all South Africans.

I strongly oppose the implementation of the National Health Insurance (NHI) Bill.While the goal of universal access to quality healthcare is commendable and aligns with constitutional imperatives, the proposed NHI system, as currently structured, is financially unfeasible given South Africa's constrained fiscal environment and competing national priorities.South Africa already provides public healthcare services to the majority of citizens, yet this sector suffers from severe mismanagement, infrastructure decay, staffing shortages, and widespread corruption. Entrusting a vastly larger, centralised fund to the same governance structures risks amplifying these issues on an unprecedented scale, potentially leading to further resource misallocation and diminished service quality.Citizens should retain freedom of choice in selecting healthcare providers and maintaining control over their personal health decisions. The private sector currently delivers efficient, high-quality care and should be leveraged as a partner rather than constrained.Government must first demonstrate effective stewardship by urgently reforming and strengthening the existing public healthcare system—addressing corruption, improving management, and ensuring reliable service delivery—before contemplating a comprehensive overhaul like NHI. Only then can public trust be rebuilt to support broader reforms that genuinely benefit all South Africans.

I do not support the National Health Insurance (NHI) Bill as currently framed, despite its stated objective of achieving universal access to quality health care. While the goal of improving equity in health care is important, the Bill presents significant structural, financial and governance risks that could ultimately weaken, rather than strengthen, South Africa’s health system.

The Bill proposes a single, centralised National Health Insurance Fund responsible for purchasing and managing all health services. Concentrating this level of financial and operational control in one entity introduces a high risk of inefficiency, mismanagement and corruption—issues that have repeatedly challenged existing state-run systems. The Bill does not provide sufficiently robust safeguards, independent oversight mechanisms or clear financing models to ensure sustainability.

Furthermore, the Bill severely restricts the role of private medical schemes, reducing individual choice and placing substantial pressure on the public sector before reliable capacity has been demonstrated. Without clear funding strategies, realistic costing, or phased capacity building, the NHI model risks overwhelming the system and reducing quality of care for all.

For these reasons, I urge the Select Committee to reconsider the Bill and engage in broader, evidence-based consultation to develop a more practical, sustainable and accountable path toward universal health coverage.

The country will fall into another corruption scheme that will be looted, and nobody will receive any Health Care. The government fails at everything besides Tax collection. So a hard no support.

In a free society people must be allowed to choose their own medical service providers. The state should never be allowed to stand between the public and the health care industry, and while healthcare must be regulated to ensure public safety, free market economics must remain sacrosanct. Anything less is to allow this government to further its communist agenda which will have no other outcome than the utter economic destruction of South Africa. The state already offers free healthcare for all. The problem is that they have destroyed service delivery through corruption and incompetence. Until they state fixes the existing health care system the NHI cannot be anything other than performative politics or a cynical money grab.
